Report a new incident

From the Incidents section, a staff member taps 'Report new incident' and the form opens. They give the incident a short title and pick the location it happened at. Reports can be submitted from the mobile app.

Choose the type and when it happened

Pick the incident type that best fits — for example an injury, property damage, or a near-miss — and set the date and time it occurred. This helps managers tell at a glance what kind of incident it was.

Describe what happened

Write a description of what happened in your own words. If anyone saw it, you can note their details in the witness field — it's a simple free-text box, so jot down whatever is useful. The more detail you add, the clearer the picture for managers.

Get the most from incident reporting

Simple habits that keep your incident records clear and useful.

Report straight away
Encourage staff to report incidents as soon as they happen. Details are freshest right after the event, so the description ends up far more accurate.
Write clear descriptions
A few specific sentences — what happened, where, and what was involved — make a report far more useful than a one-line note.
Review the list regularly
Check the incidents list periodically to spot patterns — recurring slip hazards, equipment issues, or a particular site that keeps coming up.
Use incident types consistently
Train your team to pick the type that best fits. Consistent typing makes the list much easier to scan and review later.
Always set the location
